The majority of prescriptions we dispense are on an NHS form issued to you by your general practitioner.
Repeat Prescription Service
Take the stress out of collecting your repeat prescripyion by letting the Medicine Centre do it for you. The free service is simple and convenient:
1. Simply fill in and sign the two forms and present it at the medicine centre, or post it to FREEPOST MEDICINE CENTRE
2. Order your prescription from your surgery as normal.
3. Contact the Medicine Centre to request collection.
4. Pick up your prescription from the Medicine Centre as arranged.
5. We can then detail your next repeat prescription and due date and have it ready for you to collect or we can deliver it to you
6. We will inform you of any requests by your surgery to contact them for review/ tests etc.
Our customers are exempt from paying a prescription charge in Wales.
In England
Customers can be exempt because of the following
* Are aged 60 or over;
* Are under 25 years of age (Wales only)
* Are pregnant or have had a baby in the previous 12 months and have a valid exemption certificate;
* Have a listed medical condition and have a valid exemption certificate
* Have a continuing physical disability which means you cannot go out without help from another person and have a valid exemption certificate;
* Are an NHS in-patient;
* Are getting, or your partner gets Income Support, or Income-based Jobseekers Allowance, or Minimum Income Guarantee, or Pension Credit guarantee credit;
* Are entitled to, or named on, a valid NHS tax credit exemption certificate;
* Are named on a current NHS Low Income Scheme HC2 certificate
* Are a war pensioner, the prescription is for your pensionable disablement and you have a valid war pension exemption certificate;
* Or your prescription is for contraceptives.
It is part of our contract to check the validity of the claims on prescription forms, so you may need to show us proof of exemption, as described below.
Reason for Exemption Evidence Required
If under 25 years of age
* None, if age or DOB is printed on the front of the prescription otherwise other evidence may be appropriate
Is 60 years of age or over
* None, if age or DOB is printed on the front of the prescription
has a maternity exemption certificate form FP92 or FP96
has a medical exemption certificate form FP92 or FP96
has a prescription prepayment certificate form FP92 or FP96
has a War Pension exemption certificate War Pensions Agency Certificate
Is named on a current HC2 charges certificate HC2 Charges Certificate
Gets income support (IS) Income Support Order Book, Giro letter or tear-off portion
Gets Income-based Jobseeker's Allowance (JSA (IB)) No evidence available - Your entitlement will be checked later by the DoH.
Is entitled to, or named on, a valid NHS Tax Credit Exemption Certificate
Working Families' or Disabled Person's Tax Credit Exemption Certificate
has a partner who gets Minimum Income Guarantee (MIG) partner's pension book, letter or bank statement showing payment from Department of Work and Pensions
